Western Queens Care Management
Mental Health Providers of Western Queens, INC. proudly operates two Care Coordination Programs (formerly known as case management). The programs both operate under the same name “Western Queens Care Management”. One program is for children and youth while the other program services adults. A significant percentage of the adult consumers may be receiving services through the auspices and assignment of a Health Home.
Services are for those consumers who are living with mental illness and/or serious emotional problems (Axis I diagnosis).
