Mental Health Providers of Western Queens, INC. proudly operates two Care Coordination Programs (formerly known as case management). The programs both operate under the same name “Western Queens Care Management”. One program is for children and youth while the other program services adults. A significant percentage of the adult consumers may be receiving services through the auspices and assignment of a Health Home.

Services are for those consumers who are living with mental illness and/or serious emotional problems (Axis I diagnosis). The main objective of the program is to assist these consumers in living healthy, safe and productive lives. In addition, some adult consumers may be referred for services due to their medical issues. Each consumer is assigned a Care Coordinator who sees them as needed. Services may include:

  • Linkage to entitlements, benefits and other resources
  • Referrals to behavioral health and/or medical programs
  • Crisis Intervention (24 hrs a day/7 days a week)
  • Assistance with finding employment and housing
  • Care Coordination with other providers
  • Advocacy

Western Queens Care Management can be reached by taking the #7 train to 61st Street – Woodside and walking over to the entrance on Woodside Avenue from Roosevelt Avenue. Available Queens buses include the Q18 bus, which lets you off on Woodside Avenue or the Q32, Q53, or Q70 buses, which lets off on Roosevelt Avenue.
