Child & Family Clinic Plus

Child and Family Clinic Plus is an early recognition and intervention program funded by the New York State Office of Mental Health and operated at a local level by MHPWQ, Inc.Child and Family Clinic Plus extend clinic services into the community by providing clinical services in the school setting and in the home environment.

 
Western Queens Blended Case Management
Mental Health Providers of Western Queens operates a Blended Case Management Program which is funded and regulated by the New York state Office of Mental Health. The program provides case management services to adults, adolescents and children in the queens community who are living with mental illness and/or serious emotional problems. Recipients are provided with either an Intensive Case Manager (IMC) or a Supportive Case Manager (SCM) dependent on recipient need and OMH criterial.

WQRS is a community-based program licensed by New York State Office of Alcoholism and Substance Abuse Services (OASAS).

 THE TREATMENT PROGRAM
 Western Queens Recovery Services sees alcohol and drug addiction as a holistic disease that affects the totality of an individual’s functioning; its effect on the family are equally far-reaching. Treatment goals will therefore focus on the achievement of an alcohol and drug-free lifestyle. Sustaining such a lifestyle necessitates abstinence and emotional stability as well as positive family, social, physical, vocational and spiritual functioning. Individual treatment plans must also address the needs of family members and significant others. Therefore, treatment plans will include individual, joint, family and group session as needed. A major focus of their treatment will be the fostering of the capacity to deal with life situations and enhance feelings well-being, self-esteem and autonomy. On going and active participation in the twelve-step self-help process will be stressed as being essential to the furtherance of treatment goals. 

 

Each member of the treatment team is responsive to community needs, referring agencies, employee assistance providers, and insurance companies, in order that clinically sound, affordable care is assured. Accepted Referrals: Self-referral, STARS, EAP’s, ACS, NADAP, physicians, preventive services, probation and parole, DDP, court-referrals and attorneys.

 ADMISSIONS
 Initial assessment of the Chemically Dependent Individual or a Significant Other (an individual that is negatively affected by a family member who is chemically dependent) is thoroughly performed to help determine the most appropriate treatment. 

In those cases where cessation of chemical use can be managed on an outpatient basis, patients will be assessed and admitted to Phase One of the treatment program.

PHASE ONE
Early Recovery
 Patients who enter the program are assessed to determine the ways in which genetic vulnerability, stressful life events, family tensions, and inadequate support systems may have lead to the development of the problem.  WQRS provides a comprehensive program that includes pharmacological consultation, and intensive individual, group and family counseling to help patients cope with the difficulties they are experiencing in their daily lives. 

In Phase One, the individual may participate in the following:
•    Psychiatric evaluation/ Suboxin/Naltrexone

•    Psychological assessment
•    Individual counseling
•    Group counseling
•    Introduction to the 12 Step Groups
•    Family assessment
•    Vocational Counseling
•    Nutrition education
•    Dual-addiction assessment
•    Coordination with referring person
•    Education and counseling groups for family members, including a Family Education series, Codependency and A.C.O.A. groups.

PHASE TWO
Middle Recovery
 Once patients have achieved continuous abstinence and have begun to utilize twelve-step groups and concepts, they are placed in ongoing individual counseling and counseling groups that maintain a focus on the development of the tools for life long sobriety. Much emphasis is placed on the patient learning relapse prevention management skills.

In Phase Two, the patient participates in the following:
•    Individual therapy
•    Group Therapy
•    Continued 12 Step groups
•    Relapse prevention management and Optional:

·        Middle Recovery

·        Co-Dependency Group

·        Anger management

·        Parenting Group

·        Women’s Group

 • Family/Couple therapy

PHASE THREE
Advanced Recovery
Patients may opt to continue in individual therapy and specialized treatment areas such as the Advanced Sobriety Group and the ACOA group.

 TEEN TRACK

Western Queens Recovery Services offer services to adolescents ages 15-19 who are chemical dependent. The adolescent is initially assessed by our trained, culturally competent counselors that are experienced to the need of the specific developmental stage and to the needs of their families and make their recommendation for the most appropriate treatment.

The teen track provides one individual session and two groups: a psycho-educational and a therapeutic group. Family members are supported through our Family Educational Series and Parent Support Group. Toxicology testing is monitored on a weekly basis.

 DISCHARGE
Patients undergo continuous assessment by the treatment team throughout their treatment.  Patients are discharged when they fulfill the requirements of the phases of treatment to which they are assigned.

 DWI PROGRAM
Western Queens Recovery Services provides evaluations and treatment to a 1st offense DWI client of three months with mandated random toxicology and a year long treatment for a 2nd offense DWI clients.

Western Queens Recovery Services in located at 62-07 Woodside Avenue in Woodside, Queens.  It is easily reached by New York City Subway: #7 to Woodside Avenue, two blocks to the clinic (4th floor)

Via Long Island Railroad, take the train to Woodside, exit via the stairs at the east bound platform, walk up 62nd Street to Woodside Avenue, turn left, 4th floor.

Via Car, take Queens Boulevard westbound; on the service road, turn right on 63rd street.  Go one block, turn left on to Woodside Avenue, the building is on the right-hand side of the Avenue.  Metered Parking is available
